To report a violation provisions of the Fairness To Contact Lens Consumers Act
(Public Law164.108) relating to the unauthorized filling of an expired or
otherwise invalid contact lens prescription to the Federal Trade Commission go
to www.ftc.gov and click on FILE
A COMPLAINT at the top of the page. Complete the form beginning with
"How Do We Reach You". Supply all pertinent information and in the space at the
end of the form where it asks for an explanation of the problem state the
complaint in an exact and factual manner.
